Attention Coaches! Please use the form below to submit sweatshirt/jacket sizes for your team(s). While we will do our best to accommodate all requests, specific sizes are not always guaranteed.
A separate form must be submitted for each team you register. For example, if you are registering for both Traditional and Game Day, two forms are required.
Only coaches may complete the form and all submissions are due no later than November 1st.

Please Read: Sweatshirts and jackets will be awarded strictly in the quantity equal to the number of registered athletes per team. If you submit more sizes than registrants, PSCC will make an educated determination of which sizes to fulfill and omit the remainder. To avoid incorrect sizing, provide sizes only for registered athletes.
Winning teams will receive instructions on how to purchase additional champion sweatshirts
for alternates and/or coaches, if desired.
